In order to make the most of your time and mine, please consider these will be my first questions.
- What is your name and phone number? What hours are best to reach you?
- What is your child's name?
- How old is your child?
- What grade, school and school district is she/he in?
- What part of your child's day is in regular education classes?
- What subjects do you believe your child is behind in?
- What medical conditions exist, if any?
- Has your child been bullied, disciplined, involved with the law?
- How much support do you provide to your child with school work? Does your child like all of their school experience?
- When was the most recent evaluations, and by whom (school or private)?
- Does your child have an IEP now? If yes, what is your child's educational classification? (Other Health Impaired, Autism, Specific Learning Disability, Blind/Visual Impairment, etc).
- FINALLY: Which of these concerns do you have?
- 1. My child is not making meaningful progress with this IEP.
- 2. My child's IEP is not meeting all his or her needs.
- 3. The district does not seem to be implementing what is in his/her IEP.
- 4. Other factors (behaviors, medical conditions) are getting in the way of my child's education.
- 5. My district refuses to provide my child special education services or an IEP.
